Requirements

If you are new to Bay City Public Schools and registering at Bay City Central High School you will need to know the following information:

  • If you live outside the district a School of Choice - Section 105 or 105C form must be completed and approved by Principal Marciniak.

  • If you live in the Western High School district but would like to attend Central High School a Building Choice Request form must be completed and also approved by Principal Marciniak.

You must have the following items completed, signed, and turned into our Registrar's Office before your appointment:

  1. Copy of: Transcript of student's grades from prior school

  2. Copy of: Current report card

  3. Copy of: Exit report card

  4. Copy of: Attendance sheet

  5. Copy of: Original Birth Certificate

  6. Copy of: Immunization Record

  7. Proof of Residency (i.e. driver's license, utility bill, lease, etc.)

  8. Completed: BCPS Registration Questionnaire

  9. Completed: BCPS Home Language Survey

  10. Completed: Student Residency Statement Form

  11. Completed: Concussion Awareness Form

  12. Completed: Special Education Transfer Student - Parent Permission Form (if applicable)

  13. Completed: Sports Interest Form (if interested)

  14. Transfer Information Form (included with registration packet)

  15. Transportation Busing Form (information below and include with registration packet)

You may bring your completed and signed forms with you on your appointment day, however, we prefer that you drop them off or mail them to:

If your student will need to ride a bus to and from school next year it is vital that you complete and return the Student Transportation/DayCare/Shared-Custody form for the upcoming school year included in the registration packet and found on the Transportation Department page of the District website.
